Overview of Macimorelin

• Macimorelin acetate was approved by the US FDA in December 2017 for diagnosing adults with growth hormone deficiency.

Why is Macimorelin Prescribed? (Indications)

Macimorelin acetate is a growth hormone (GH) secretagogue receptor agonist which is prescribed to diagnose adult growth hormone deficiency (AGHD).

When should Macimorelin not be taken? (Contraindications)

Macimorelin acetate should not be used in the following conditions-

• Allergy to macimorelin
• Children
• Body mass index (BMI) above 40 kg/m2
• Deficiencies of sex hormones, thyroid hormones and/or glucocorticoids
• Patients taking drugs that can affect pituitary GH secretion

What is the dosage of Macimorelin?

• The recommended dose is determined based on the individual patient’s body weight.
• It is taken as a single dose of 0.5mg/kg body weight after a fasting period of 8 hours.

How should Macimorelin be taken?

Macimorelin comes as granules in the 60mg sachet which has to be reconstituted with 120 ml of water and should be taken by mouth.

Patients weighing below 120 kg - Use 1 sachet
Patients weighing above 120 kg - Use 2 sachet

• The reconstituted solution should be used within 30 minutes after preparation and the unused solution should be discarded.

Test procedure:

• The recommended dose of macimorelin granules can be determined by multiplying the patient weight in kilogram (0.5mg/kg).
• Dissolve the contents of the sachet appropriately in the required volume of water and stir the contents gently for 2 to 3 minutes to obtain a final concentration of 0.5mg/ml.
• An appropriate volume of the solution can be transferred by using a syringe without the needle into a drinking glass which should be drunk in 30 seconds.
• The blood samples should be withdrawn at regular time intervals of 30, 45, 60 and 90 minutes after the administration of macimorelin oral solution to determine the concentration of growth hormone.
• If the serum growth hormone level is less than 2.8 ng/ml at all the measured time points after macimorelin administration, the test result confirms the presence of adult growth hormone deficiency.

What are the warnings and precautions for Macimorelin?

• It is necessary for a patient to undergo fasting for about 8 hours before taking macimorelin oral solution.

• Patients with cardiac illness should be monitored closely to avoid the risk of developing abnormal heart rhythm which can even lead to sudden cardiac death.

• Repeat testing may be required in patients with lesions in a part of the brain called the hypothalamus as there is a chance of a false negative test result.

• Reconstitution should be done only by a trained healthcare professional and should be administered only under medical supervision.

• The use of macimorelin in pregnancy and breastfeeding has not been established.

What are the side effects of Macimorelin?

Gastrointestinal: Increased appetite, nausea, diarrhea, increased hunger, taste disturbances
Cardiovascular: Abnormal heart rhythm or prolonged QT interval on ECG
Nervous system: Headache, dizziness, fatigue
Respiratory: Infections of the respiratory tract
Skin: Excessive sweating, feeling hot

What are the other precautions for Macimorelin?

Patients should be treated on an emergent basis if the following severe allergic reactions occur:

• Abnormal heartbeat
• Chest tightness
Wheezing
• Fever
• Itching
• Swelling of the face or throat
• Unusual skin coloration

What are the Drug Interactions of Macimorelin?

• Macimorelin can lead to the development of abnormal heart rhythm when administered in patients receiving drugs like quinidine, procainamide, amiodarone, sotalol, moxifloxacin and antipsychotic drugs such as haloperidol, chlorpromazine, ziprasidone or thioridazine and therefore should not be taken together with any of these drugs.
• Drugs like phenytoin, carbamazepine, enzalutamide, rifampin, efavirenz, modafinil, rufinamide, bosentan, St. John’s Wort reduce the serum concentration of macimorelin and can lead to false positive results of the test.
• Drugs such as insulin, glucocorticoids, somatostatin, aspirin, indomethacin, clonidine, levodopa, atropine, propylthiouracil affect the growth hormone release and can interfere with the test result.
• Stop the intake of growth hormone products at least one week before the administration of macimorelin.
