Tobramycin has a narrow antibiotic activity, and is used to treat life-threatening infections and it is mandatory that effective levels be achieved without overdosing.
Tobramycin is cleared by the kidney and accumulates in renal tubular cells. Nephrotoxicity is most closely related to the length of time that trough levels exceed 2 µg/mL. Creatinine levels should be monitored every 2-3 days to detect renal toxicity.

Tobramycin is potentially toxic to the nerves and the kidneys. It may affect the nerve to the ear resulting in hearing loss, dizziness and vertigo. It may also cause tingling and numbness, muscle twitching and even seizures. It should be avoided in patients with kidney dysfunction. It could cause respiratory difficulty in patients undergoing anesthesia or those with myasthenia gravis or Parkinsonism and therefore should be used with caution in these patients.
